Can inactive ingredients in Synthroid cause symptoms?
Yes. Inactive doesn’t mean your body can’t react to it. It means the ingredient isn’t the thyroid hormone.
Synthroid’s own prescribing information lists reactions to inactive ingredients, including hives, itching, swelling, and digestive symptoms. If you started feeling worse after a medication change, I’d look at the complete ingredient list, not just the dose. [1]
What else changes when my Synthroid dose changes?

Does the white 50 mcg Synthroid tablet have fillers?
Yes. It’s dye-free, not filler-free. It still contains lactose, acacia, confectioner’s sugar with corn starch, magnesium stearate, povidone, and talc.
If you’re trying to avoid lactose, choosing the white tablet won’t solve that problem. [1]
Are the fillers in generic levothyroxine the same as Synthroid?
Not necessarily. Both contain levothyroxine, but the fillers depend on who makes the tablet.
Start with the manufacturer on your prescription bottle. Then have your pharmacist compare that product’s full ingredient list with Synthroid’s. The word ‘levothyroxine’ tells you which hormone you’re taking. It doesn’t tell you everything else in the tablet. [1][9]
How do I know if it’s a filler reaction or the wrong dose?
Start with what changed. Did your symptoms begin after a different strength, a brand switch, or a refill from a new manufacturer? Write down the timing and keep the old and new bottles if you have them.
I’d take that comparison and your recent thyroid labs to your prescriber. You want to investigate both the hormone dose and the ingredients rather than assume every new symptom means you need more thyroid medication. [1][9]
Can I avoid a dye without changing my total dose?
Sometimes. Your prescriber may be able to use a combination of tablets that avoids the dye while keeping your prescribed total dose the same.
Bring the name of the dye you want to avoid and ask your pharmacist to check the options. The goal is to change the ingredient exposure without accidentally changing how much thyroid hormone you take. [1]
What if my reaction isn’t on the usual side-effect list?
That doesn’t automatically rule out your medication. One published case describes swelling around the eyes that resolved after the patient switched to Tirosint. The report didn’t establish which ingredient caused it.
I’d use that as a reason to investigate an unusual reaction, not dismiss it. Photograph visible changes and write down when they happen so you have something concrete to bring to your appointment. [9]
